Florida requires LLC names to be distinguishable. Give us a primary name and up to two fallbacks — we'll check availability and use your top available choice. Each must contain "LLC", "L.L.C.", or "Limited Liability Company".

Florida requires a Registered Agent with a Florida physical street address (no P.O. box). Maua can provide one for ~$125/year if you don't have one.
Everyone who owns part of the LLC. Florida does not publish member names, but we need them for the Operating Agreement, EIN, banking, and tax filings. Ownership percentages must add up to 100%.
Banks usually require at least one. Use AMBR for Authorized Member, MGR for Manager.
How would you like the LLC to be taxed? When in doubt, leave the default — most owners keep the default classification.
